Title: Manager, Community Health Systems & Benefits Navigation
Pay: $30/hour
Location: NYC
Duration: Long term contract
We’re partnering with a mission-driven organization focused on advancing health equity across NYC communities.
They’re hiring a Manager, Community Systems & Navigation to support a network of community-based organizations (CBOs) in building the systems, skills, and workflows needed to deliver benefits navigation and enrollment services—specifically under the 1115 Waiver. This is a highly impactful, hands-on role working directly with community partners to strengthen access to critical services.
Build Capacity & Drive Impact
- Partner with CBOs to assess gaps in benefits navigation, enrollment, and service delivery workflows
- Design and deliver practical, real-world training (train-the-trainer, workshops, tools)
- Provide hands-on support through coaching, site visits, and small group sessions
- Help organizations integrate benefits navigation into existing programs and case management systems
- Strengthen CBO readiness to conduct benefits screening, enrollment, recertification, and workflow integration
- Translate complex policy changes (Medicaid, SNAP, 1115 Waiver) into clear, actionable guidance
- Ensure partner readiness ahead of major milestones like benefits cliffs and work requirements
- Support onboarding of new partners and scaling of navigation capabilities
- Guide partners in tracking key metrics (screenings, enrollments, recertifications, referrals)
- Help teams use data to strengthen service delivery and community impact
- Work cross-functionally with internal teams to align strategies and identify gaps
- Build strong, trusted relationships with community partners
- Facilitate peer learning and sharing of best practices across organizations
- 2–3 years of experience in community health, human services, or public sector programs
- Hands-on experience with benefits navigation, Medicaid and/or SNAP enrollment
- Strong understanding of health equity and social determinants of health
- Ability to break down complex policy into clear, practical guidance
- Experience training, coaching, or supporting frontline teams or community organizations
- Strong project management skills—you can juggle multiple priorities and keep things moving
- Excellent communication skills with the ability to engage a wide range of stakeholders
- 1115 Waiver / Social Care Networks
- Case management or referral systems
- Certified application counselor workflows

The individual may also be eligible for discretionary bonuses.
