? Build strong, trusting relationships with Medicare members, grounded in empathy, respect, and patience.
? Day-to-day workload communicating with members through in-bound calls, out-bound calls, and member inbox. Member communications can take place through video calls, audio-only calls, text messages and emails.
? Connect members to community-based social services (such as food assistance, transportation, housing support) to address health-related social needs.
? Support appointment scheduling and healthcare access, including provider lookups and reminders.
? Help members become stronger self-advocates in managing their health and care.
? Participate in a schedule including nights, and holidays, for immediate response to emergency-level cases.

The individual may also be eligible for discretionary bonuses, and can participate in medical, dental, and vision benefits as well as the company’s 401(k) retirement plan.
